I replaced my Park Tool nipple wrench.

It's the SW from the Spoke Wrench line.

The main reason for replacing it was the wear on the gripping surfaces—the part that engages 3 out of the 4 faces of the nipple.
But the grip itself gets worn down by the spokes too, which is another sign it's time.
And honestly, the grip has gotten pretty thin.
A new tool just feels good in your hands.
